I am trying to source training for a couple of guys in RCFA. Can anyone help me to identify companies that provide Root Cause analysis training courses in the UK. I would prefer this training to be tailored toward machinery and equipment failures. The training should provide training in the various techniques e.g. 5 why's, 8Ds, Causal trees etc. (not just awareness or overview training).

If you are still interested in specific RCFA training, then GE Energy's Reliability Consulting & Implementation Services could look at your requirements and design a specific training course to match your needs..... let me know if you are interested and we can discuss further.
